Payment is due in full at the time of service.

- Visa/Mastercard
- Patient Co-Pays and Deductible plus Acceptance of the "Assignment of your Insurance Benefits" (note: assignment of benefits form must be signed).
- Unicorn Financial Program (monthly payments financed up to 5 years). Ask the front office for the details.
Bookkeeping Rewards
- 10% Reward - prepayment of entire fee *
A minimum of 48 hours prior to service (cash or check)
- 8% Reward - prepayment of entire fee *
A minimum of 48 hours prior to service (Visa/Mastercard)
- 6% Senior Reward (62 years old qualifies / cash, check, Visa, or Mastercard)
- 5% Reward - paid in full * (cash, check, Visa, or Mastercard)
One Reward Per Transaction
*Even if you have dental insurance, it is acceptable to take advantage of the 5%, 8%, or 10% discount.
Simply make full payment minus your bookkeeping reward and we will file your insurance forms and direct your insurance company to send payment directly to you.
